Birth Records since October 1907. The non-refundable search fee includes (1) certified copy if found. For records prior to 1907, write to the Health Officer in the city or county where the event occurred.

Death Records since 1900. The non-refundable search fee includes (1) certified copy if found. There are no indexes to the state death certificates form 1900 through 1918. The specific city or county of death must be provided for searches in this period and the search is limited to one county for each $4.00 search fee paid. For records prior to 1900, write to the Health Officer in the city or county where the event occurred.

Certified copies of marriage certificates are not available from the State Health Department. They are available from the Clerks of the Circuit Court in the county where the marriage was granted.

Certified copies of divorce certificates are not available from the State Health Department. They are available from the County Clerk in the county where the divorce was granted.
